Changes to the Professional Practice Experience Form (PPE) and Practice Questionnaire (PQ)

Date Posted:
August 15, 2016

Updated versions of the Professional Practice Experience Form (PPE) and the Practice Questionnaire (PQ) have been approved by the Qualifications Review Committee (QRC)

Effective January 1, 2017 applications will be required to include the new forms, and the old version of the forms will no longer be accepted.

Both forms are part of the application submission for the Canadian Registered Safety Professional (CRSP)® certification.

An applicant would complete the PPE for each position about which they are reporting for credit in their application submission. The form requires the applicant to describe the job responsibilities with an emphasis on the OHS duties. In addition to the PPE, an applicant is also required to submit an employer verification letter and the formal job description for each position.

An applicant’s direct supervisor completes the PQ form and must be submitted directly to the BCRSP office by the reference.

Applications that have been received by the BCRSP office, but have not been submitted to the QRC for review by the January 1, 2017 deadline due to missing documentation, will be required to submit the new versions of the PPE and PQ.

The new forms are available on the website at the following links:
