Application Forms

The CRSP eligibility criteria changes July 1, 2018. Click here to find out more.

Registration Process
To qualify as a Canadian Registered Safety Professional, an applicant must successfully complete a program of registration that comprises six phases generally carried out in the following sequence: application, evaluation, interview, examination, approval and confirmation. The process is governed by formal registration procedures that have been designed to provide a fair and honest appraisal of each person who applies. All matters respecting an individual's progress are treated on a personal and confidential basis. The period of time that is involved from application to final confirmation may take from several months to a year or more.

Applying For Registration
To apply, an applicant completes an application form and, together with a non-refundable application fee, forwards it to the Board office. The form requires details concerning formal education, professional development and related experience. In addition, a reference questionnaire and practice questionnaire must be completed by individuals who have a sound knowledge of the applicant's work performance.

Application Form
The applicant is cautioned not to proceed with application submission unless the minimum eligibility requirements are met. When completing the application, ensure that documentation is attached for each factor (see application checklist). Ensure that the persons selected to complete the questionnaires are aware of the necessity to complete and forward the forms accurately and promptly as failure to receive all of the questionnaires will delay the registration process.

It is the applicant’s responsibility to ensure that all pertinent information is submitted with the application along with the required documentation.

Please note that commencing January 1, 2018 one of the two questionnaires (practice or reference) must be completed by a CRSP or equivalent (such as CSP, CMIOSH, CIH, ROH, PEng, CHRP/CHRL).
